The person-centred care plan is carried out with the resident. If this is not possible due to the resident being incapable of participating then a member of their family or advocate is always involved and signs the care plan when it has been agreed.

The care plan is a comprehensive assessment of all aspects of their health, personal and social care needs.

Our care plan sets out any problem that the resident has, then a goal of care for the problem, followed by details of how to reach the goal. It is straightforward but fully comprehensive.

The reviewing process incorporates the resident’s weight chart followed by a needs assessment of whether the care plan needs updating. The residents are weighed monthly and therefore the care plans are updated monthly. If a plan needs updating sooner or if it is ongoing, then the care plan is changed accordingly.
